US government mislabels countries on map of Africa at global conference

A significant cartographic error by US State Department officials at an international health conference in Brazil drew widespread public attention and criticism online. During the AIDS 2026 conference in Rio de Janeiro, attendees recorded and photographed a map of Africa displayed during a presentation on new US health agreements with African nations. The map contained multiple substantial errors, including the placement of Nigeria in the Sahara Desert, relocation of Mozambique from southeastern Africa to the Horn of Africa region, and positioning of Ivory Coast on the opposite side of the continent from its actual location. Country boundaries on the map appeared as indistinct shapes rather than accurate political divisions.

The presentation was delivered by Jeff Graham, the top US health envoy who heads the President's Emergency Plan for AIDS Relief (PEPFAR). Graham's previous diplomatic postings included Ghana and Ethiopia, according to his official State Department profile. The map's appearance during discussion of new health initiatives with African partners generated thousands of online views and substantial public ridicule across social media platforms.

Investigation by Reuters news agency identified a watermark on the map indicating it was created using OpenAI tools. OpenAI subsequently announced it would investigate the incident. The US State Department issued a statement acknowledging the error, describing it as produced by a team member who hastily modified presentation slides immediately before the event. Officials stated they accepted full responsibility for the confusion and misrepresentation experienced by conference attendees and African partners, while characterizing the substantive discussions as constructive despite the cartographic failure.

The incident occurred amid broader changes to US HIV assistance programs. The Trump administration previously implemented a pause on HIV funding pending review, affecting PEPFAR's operations which had supported approximately 20 million people globally with antiretroviral treatment. While core PEPFAR activities have resumed, the US continues scaling back HIV prevention and surveillance initiatives. The administration has also announced plans to terminate the program in South Africa, which carries the world's largest HIV disease burden.
